Piriformis syndrome

Is it sciatica, or is it piriformis syndrome?

Both, usually. G3 Chiro in Newport Beach treats piriformis syndrome, which is sciatic nerve irritation that happens in the glute rather than at the spine. Sciatica is the symptom, and the piriformis is one of four places along the nerve that can cause it. The sciatic nerve passes through a fascial tunnel underneath the piriformis muscle, and tension there, often driven by a stuck hip or sacroiliac joint, closes that tunnel. Dr. Jaron Hua tests the nerve along its whole route rather than assuming the spine, in sessions of 45 to 90 minutes.

Does this sound like you?

  • A deep ache in one glute you can never quite press on
  • Worse after sitting, driving, or sitting on a wallet
  • Pain or tingling down the back of the leg, but your back feels fine
  • A hamstring that feels tight no matter how much you stretch

Why it keeps coming back

Most sciatic pain is worked up at the lumbar spine. Imaging finds a disc bulge and the disc becomes the story, even though large studies repeatedly find people with disc bulges and no symptoms at all. Fascia, the tissue most likely to be closing the tunnel at the piriformis, barely shows on an MRI. It only shows up in a hands-on exam that tests the nerve along its whole path.

How Dr. Jaron looks at it

Dr. Jaron runs a neurological screen, then nerve tension testing to isolate where along the route the restriction sits, then assesses lumbar and sacroiliac motion and examines the fascia by hand. Treatment frees the nerve along its line rather than pressing into it, restores glide before any stretching, then loads the posterior chain once the same tests come back clean.

What to expect

An assessment that rules each of the four possible sites in or out rather than assuming the spine, hands-on treatment, and exercises matched to the site the exam actually found. The same tests are repeated so progress is measured rather than guessed.
